CHAPTER PROJECT SIGN UP

Unsponsored Service Opportunities:

Please fully complete this form at least 48 hours before the event takes place to be reviewed by the executive committee. You must receive an email from jthhonorsociety@gmail.com saying that your event is approved to log hours for the event. You must complete this form to have your unsponsored approved. Otherwise, your service hours will not be approved.

Validation Forms:

This year we are moving to only electronic validation forms. Please fill out THIS FORM to confirm and submit your hours. It is YOUR responsibility to fill out a validation form for each event and take a picture of yourself at the event for evidence. If a form is lost and you do not have a digital record of the form, then your service hours will not be accepted.

 

Individual Service Project: 

Individual Service Project 12 W's

These projects must either be your own individual idea and not through another club, organization, non-profit, etc. They must also be a group project that is accessible to all NHS members. If you organize a group project, you must reach out and ask if the organization needs volunteers before we can approve it. If it does involve another entity, please have them email your adviser to follow a different path. You could receive additional hours for organizing a group event, if justifiable.
